Van Use Reservation
If you use the van, please abide by the following rules. You are essentially 'renting' the van for the use of the ministry you are serving.
- Please be sure OSLC has a copy of your driver's license on file.
- Use the mileage sheet to document the date, driver, ministry, and mileage (found in the mailroom).
- Please fill up the gas tank if it is half full or less by the end of your trip. You can obtain a credit card from the church office.
- Note on the mileage sheet that gas was purchased and the cost before returning sheet to mailroom.
